In 1986 I got married to Jane and for the first 4 years ejaculation was painful. We cried many tears. Don't know what changed, but it went away. But I don't think I've ever had orgasms. But we made it for 24 years. Now in the past 5 years it seems like problems are getting worse again. It started out 5 years ago sexual sensations declined more and more. Two years ago it really started to decline. It started out with an "over the hill" feeling----losing interest & desire, and full prostate, but somewhat difficult to get an erection. This episode was after a 4-day wait. Now in the past 6 months to a year this scenerio is 4X worse. Now if I don't get released every other day I get a full prostate, and it makes me sick, such as: no energy, jittery, hypertension, restlessness, can't sleep, heart pounding, wake up early, can't hardly work, because this "big, bad guy" is fighting against me; I have to fight with all my might to stay on top; no energy left for anything else; can't think, beside myself, grouchy & frustrated---completely out of character for me; if I take a nap it relieves it some; I get dull headaches, low back pain if it goes too long; don't think I ever have orgasms; as I near ejaculation the tip of my penis on top gets very, very tender; and, also, gets bright, bright red and red polka dots (looks like petechia or something); if I rub it with my thumb with moderate pressure over the whole area anywhere on the tip/top it hurts with a dull pain---not a stabbing pain, but it makes ejaculation a painful experience/relationship. 7-28-09 my testosterone level was 151. I have taken testosterone gel. Then 10-27-09 my level was 304. Dr. wanted it higher so he gave me shots. 2-15-10 my level was 567. As the testosterone level came up I had more desire for sex, but it seemed like the prostate was manufacturing semen terribly much faster and I walked around miserable most of the time with these feelings. I finally quit and my testosterone went back down to 150. My prostate doesn't make as much semen now, but if I don't get a release every other day I immediately start going downhill. And it gets bad fast. There's hardly any sensation in my penis any more. It feels 3/4 numb, which makes it hard to get an erection. The desire only lasts about 15 minutes to half an hour. I went to see 2 urologists. The one said "I don't know"; the other said "Let's try to give you some medicine to slow your prostate down." (Proscar 5 mg, 1 daily) Sounds like a bandaid on a broken leg to me. I have browsed & studied a lot on your website; I wish I could have read this 24 years ago. But I'm still wondering where to go next. It seems like there's a physical problem causing the numbness in my penis. It feels like a dead piece of sausage. This problem alone takes most of the fun out of it. Then the pain on ejaculation ruins the rest of it. Let alone the bad feelings in between. Next, I'll answer your questions. I've always had super vision until 6 months ago! . Now reading a book with fine print at 18" is blurry and getting worse. Have some memory loss, some frequent urination. Sometimes I feel somewhat depressed. Hypertension if prostate gets full. If it goes too long I get testicular pain and vas deferens sometimes hurts, also. Don't smoke, chew, do drugs, or drink bud-dumber, etc. My wife and I both agree I need some help. Can you help me? All of your problems result from androgen deficiency, a lack of DHEA, testosterone and DHT, leading to weak immune and chronic inflammation.
In addition to fueling sex, semen production and orgasm, androgen hormones are essential to the penile nervous sensitivity, modulation of inflammatory genes HLB-B (human leukocyte antigen (HLA) major histocompatibility complex, class I, B) which have a most troublesome gene HLA-B27 (subtypes B*2701-2759),  suppression of inflammatory cytokines and COX-2 enzyme expression, and parasympathetic nervous nitric release in the arterial wall for relaxation and dilation.

Testosterone shot is not a good therapy since it gives a testosterone surge in the first couple days but wan out in few days.
If possible, get a prescribed testosterone gel for a balance treatment.
