cause and solution for vaginismus and sexual orgasm

HELP!!!!!
My wife and I love each other very much (she is my soul partner, for which I am very blessed). Our problem has been going on for several years and we have finally reached a point where we need some answers and/or advice.
After several female medical problems and surgeries for endometriosis my wife had to undergo in order to level out her hormones, thyroid and endometriosis, we are left with the following situation:
My wife experiences pain when I fully enter her (she has expressed that is feels like she is going to explode similar to putting a pencil lead through the eye of a needle). We have tried various positions, but none seem to help the problem (some of them were just too painful for her)
We have tried several gels, lotions and even numbing creams (the cream allowed me to enter her, but it completely blocks her from achieving an orgasm). We have tried to "replace" me by using varios dildos, oral sex and whatever she thought might stimulate her. It took several purchases to find a dildo small enough to not cause pain, but this lacks the "closeness" and satisfaction that we so much want to share together.
We are looking into vaginal reconstruction surgery as a possible option. We have tried several other options with no success so far (vaginal stretchers, etc.) We need to know the risks and possibly get references of those who have had this done in order to decide. If there is a place to get information and do our own research we would very much like to know what it is.
My wife has had a long battle with her hormones (her skin used to crack and itch in her vaginal area). With the use of DHEA, Estr?ce cream, Unithr?id and Testosterone, her skin condition has greatly improved. The outer area is approx. 80% back to normal. The interior is now our concern.
My penis is a litte bigger than the average size, but I refuse to believe (as one of our doctors quoted) that I am just too big and she is just too small.

We are looking for a viable option or any advice you can offer.

This is called vaginismus due to a lack of the tissue-/nerve-relaxation/elasticity hormone Prostaglandin E-1 (PGE-1)synthesis by the vaginal tissues. Without PGE-1, the vaginal nerves can not stand the stress and strain produced by penetration.
Elevation of both the hGH/DHEA/androstenedione and the neurotransmaitter acetylcholine will solve her problem.
